Patient with Dementia:

Profile:

- Moderate dementia

- Moderately severe cognitive decline: May not remember everyone's names, their own address or their number

- Needs help in performing day to day activities, like bathing, eating and going for walks.

- Deals with sundown syndrome, wherein the patient gets moody during the evening time, and may still think it is morning.

- Irrational and unexplainable changes in behavior and mood

Value stories:

As a patient with dementia who likes reading stories, I want someone to read stories to me, so that I can keep up my hobby despite my illness.

As a patient with dementia, I want day to day activities to be as interesting as possible so that I can do them for my wellbeing.

Nurse

Profile

- Stays at the patient's house for 24x7 support to the patient

- Has medical background on care that the patient needs

- Knows patient's medical history

- Knows how to support patient in day to day activities

Value story:

As the formal caregiver, I need to support the patient in performing day to day activities.

Family Member

Profile: 

- Patient stays with this family member at their house

- Wants the best for the patient, and wants to do everything they can to provide the best care

- Worries about the patient's health and whether it may deteroriate

- Works a full time job to provide for the family and patient's medical needs

As the informal caregiver, I want the patient to have the best possible care, in terms of their mental and physical wellbeing.
